Generic Compliance Ratio: How to Calculate Your Pharmacy's Secondary-Wholesaler Budget
Empire Rx Distributors · 5 min read
The clause that decides how much you can shop around
Most prime vendor agreements tie your generic rebates to a generic compliance ratio (GCR): the share of your generic purchases — by dollars or by eligible lines — that must go through the primary's source program. Common thresholds sit somewhere in the 85–95% range, and the rebate tiers step with it. The number is in your agreement, and your rep can tell you where you stand today.
Find your real number, not your assumed one
Pharmacies routinely assume they are at 100% when they are already at 92%, or assume they have room when a slip would cost a rebate tier. Pull the primary's compliance report (most portals publish it monthly). Compare it against the threshold in your contract. The difference, multiplied by your monthly generic spend, is your secondary budget.
A worked example
A store spending $60,000 a month on generics with a 90% compliance requirement can direct up to $6,000 a month to a secondary without touching its rebate tier. If the secondary is 15% cheaper on the items it wins, that is roughly $900 a month — about $10,800 a year — in cost of goods, from purchases your contract already permits.
Spend the budget on the right items
The budget is finite, so put it where the gap is largest: high-velocity generics where a secondary's price per unit is clearly lower, and short-dated deals that fit your dispensing speed. Do not spend compliance room on brand products (your primary almost always wins there) or on slow movers where the savings never add up.
Watch the ratio monthly
Set a calendar reminder: pull the compliance report, confirm you are above threshold, and adjust next month's secondary orders. If you drift close to the line, pause secondary buying for a week — a lost rebate tier erases months of savings. A secondary with no contract and no minimum commitment makes this easy to dial up and down.
