7 Ways Independent Pharmacies Lower Cost of Goods (Without Switching Primaries)

1. Know your top 50
A few dozen NDCs usually drive the majority of your generic spend. Export your purchase history, rank by dollars, and focus every tactic on that list. A 10% saving on a top mover is worth more than 50% on something you buy twice a year.
2. Use your full secondary allowance
Your prime vendor agreement's generic compliance requirement leaves a slice of purchasing you're free to place anywhere. Most stores don't use all of it. Price your top 50 against a secondary monthly and move the clear winners — that alone is often the largest available saving that requires zero negotiation.
3. Buy short-dated on your fast movers
Product with 6–12 months of dating is steeply discounted because the distributor's alternative is destruction. On an item you dispense weekly, that dating is all you'll ever use. Match dating to your dispensing velocity — confident you'll dispense it in half the remaining dating — and the discount is pure margin.
4. Check the unit price, not the invoice price
A bigger bottle isn't automatically cheaper per tablet, and two suppliers' "same" item can differ in pack size. Compare per-unit cost at the NDC level. Any supplier worth using shows pack size and per-unit price without making you do the division.
5. Mind the payment terms
Net terms are part of the price. A 2% cash discount for prompt payment, where offered, is one of the highest risk-free returns available to a pharmacy. Conversely, carrying past-due balances quietly costs you: suppliers price risk into accounts that pay slow.
6. Return in-date overstock on time
Money on the shelf you'll never dispense is cost of goods too. Calendar a monthly sweep for slow movers still inside the returnable window (typically 12+ months of dating) and send them back for credit before they age past it.
7. Make reordering cheap in time, not just dollars
